When M and Bond are at discussing Mitchell at his house, Bond says something about "you passed him on the stairs" and "you're lucky" to be alive. What/who is he referring to?
I was under the impression that there was another bad agent and that Mitchell had killed him while "securing the perimeter" and had in fact saved M's life.

I think Mitchell killed the security guard on the stairs to allow White to escape safely. Not to sure about the "Lucky to be alive" comment though. I think Bond was reffering too how he pushed M out of the way, to avoid Mitchell shooting her.

This scene was edited in a way to create confusion for the audience, as it looked like M got shot. It reminds me of a similar edit when Bond evades M and guards in LTK, where for a moment it looked like Bond struck M, but didn't.

Mitchell went to check the perimeter, but in fact was going to kill the MI6 agent guarding the building. When M fled, she passed the dead guard on the said, unknowing.
She's lucky to be alive because Mitchell very deliberately tried to kill M along with the other agents in the room.

"You passed him on the stairs" refered to one of M's bodyguards, that Mitchell killed during his "perimeter check".
"You're lucky to be alive" refers to the fact that M WAS shot by Mitchell, but it was only a raspy shot (i'm sorry if i'm don't know if i'm making myself clear).

AH ok. I was making it more than it was.
I was thinking that Vesper->Bond as Mitchell->M.
