Madonna's cameo according to all I've been told includes;
1) No dialogue
2) Two seconds of screen time.
And yes a cameo Ronnie, by it's nature it's a secret wink and the audience. Secret winks aren't credited!

Is she doing this job out of generosity? No, EON/MGM threw a ton of cash at the do-anything-for-money-dancing-monkey and her agent said yes.
If EON had asked her back in the 80's, before she went all respectable, it would have been a daring choice and I'd have been impressed. But this is 2002 and Madonna's wearing her safe,establishment face.
Besides, Austin Powers got her first. Bond shouldn't be lumbered with some other spy's cast-off

Remember everyone that the credits are one of the last things to be put on the film. They are probably not done yet, heck they're probably not even started (filming-wise, that is). I am sure that with Halle Berry's Oscar win she will be billed second in the opening credits and in the U.S. poster campaign. Stephens may take the second slot in the U.K. poster campaign, hence the second spot on the call sheet. I seem to recall seeing on the site the cast listed in the order I've been guessing, that is Pierce Brosnan, Halle Berry, Toby Stephens, Rosamund Pike, and Rick Yune. I'm thinking Will Yun Lee could get a sixth credit the way Robbie Coltrane did in GE. Possibly.
