when Bond is caught
Blofeld tells Bond his escapades have revealed his true identity; he also reveals a blunder that the real Hilary Bray would not have made (Bond identified the wrong church as the repository of the de Bleuchamp birth records)
the church Bond mentions is Alsberg or something similar, this is the same church name that the real Hilary Bray tells him before he meets Blofeld.

Bond makes an assumption that turns out not to be correct.

Bond's cites the wrong location - Augsburg - for the Bleauchamp tombs and this is the superficial reason he is detected by Blofeld as an impostor.
There is a lot of confusion about this so called blooper. After some *serious* research into this I believe there is no blooper in the film at all.
When Bond is at the College of Arms - Sir Hilary Bray simply says:
"You might try inviting him to Augsburg - the De Bleuchamps' have been coming from there for generations."
This is a true statement. There is no disinformation here.
Later when Bond (acting as Sir Hilary) has his initial meeting with Blofeld the exact dialog is:
BOND: Oh one more thing - it would be very helpful if you could accompany me on a visit to Augsburg.
BLOFELD: Augsburg?
BOND: The ancestral home - of the De Bleuchamp family. There are notable Bleuchamp tombs in the Cathedral and some important records in the city archives.
As far as we know Sir Hilary never told him where the tombs were. The implication is that Bond derived this information as he studied the documents - he jumped to a conclusion before he finished all the research. Blofeld gives him a suspicious look and tells him to continue his work.
After Bond is caught - Blofeld says:
"On the other hand they do get their professional details - right! The De Bleuchamp tombs are not in the Augsburg Cathedral as you said, but in the Saint Anna Kirche"
Kirche is German for church. The Church of Saint Anna am Lehel is only miles away from the Augsburg Cathedral. To me, its crystal clear - Bond screwed up - there is no blooper! I think the real reason he is exposed is due to his indiscretions with the female patients...

Blofeld deliberately sent his letters to the College of Arms to trap Bond, who would obviously be assigned to the case, and who he wanted dead because of YOLT. Once SPECTRE knew Bond was on the case (tipped off by an agent from within MI6), agents went round to the College and told them that they were form the Ministry of Works and there was a gas leak - everyone would have to go home for the day. They then installed one of their agents in the office of Sir Hilary Bray. The man had been trained for months to sound like a pompous upper-class Scot, and knew all the heraldry stuff inside out. 'Bray' then suggested that Bond take his place and travel to Piz Gloria...
Do I get the prize for fanwankery of the day?

That part had always slightly bugged me and, while it doesn't detract from a super film, I can now enjoy OHMSS just that little bit more

While I agree with doublenoughtspy's explination, this explination is exactally the same way Kronsteen attempts to trap bond with the decoder in FRWL.

Clears it up for me too. The only reason I didn't bring it up in my article was I wasn't sure of the real reason and I generally try not to mention something problematic if I don't have either an answer for why it the way it is or a solution to it.

I just passed it off by assuming the real Sir Hilary was a SPECTRE agent giving false infomation to trap Bond
